Health Information Management

The HIM Department is committed to the quality, confidentiality and security of health information. To request medical records, contact an HIM associate during regular business hours Monday through Friday. Our associates will assist you in obtaining copies of your information. By calling ahead to request the information, you may avoid unnecessary waiting.

For continuity of care purposes, records can be sent directly to the healthcare provider you designate which eliminates the need for you to make a trip to our office.

Medical information will not be released over the telephone.

Confidentiality and security of health information is taken very seriously by the HIM Department. A photo identification is required to obtain a copy of your medical record. No one, including your spouse, can sign a release for your records. Legal documentation is required to obtain records on a deceased patient and also with any other legal situations.
